HealthCare Support is actively seeking an RN Case Manager to fill an opening with a large Healthcare Network in Hauppauge, NY.

Daily Responsibilities for RN Case Manager:

  • The RN Case Manager works in a remote and office/field-based capacity while managing their assigned home care patients care plans
  • The case manager will assess patients, create and manage care plans, work with home care aids on implementation of care plans, and complete the necessary administrative tasks

Required Qualifications for RN Case Manager:

  • Active RN License
  • 3+ years of experience in Healthcare, Elder Care, Social Work, or related field
  • Recent field experience or home care experience
