Clinical Pharmacist - Pediatric

Novant Health is seeking a Pediatric Clinical Pharmacist, to join our acute care pharmacy team, to deliver optimal medication management across all dimensions of care, every time.

Preferred Qualifications

-Clinical pharmacy practice experience with pediatric ICU and general pediatrics

-Formulary and medication order set review

-Graduation from PGY2 Pediatric Residency

Essential functions include :
  • Will work exclusively in pediatric units
  • Facilitates pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence- based best practices and regulatory standards
  • Completes critical patient monitoring and reviews patient profile/ chart to identify, prevent, or mitigate drug- related problems,
  • Partners as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient- centered care
  • Makes appropriate evidence-based, patient- centered medication recommendations
  • Participates in the management of medical emergencies
  • Provides discharge medication review, reconciliation, and counseling as appropriate
  • Maintains proficiency in and actively engages in central dispensing functions and operational programs
  • Oversees pharmacy operations and technician practice/activities
  • Maintains proficiency in preceptor roles and actively emulates education/ mentorship skills

Qualifications

  • Education: 4 Year/Bachelors Degree in Pharmacy, required. Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification, required. PhD, preferred. Doctorate of Pharmacy, preferred.
  • Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.
  • Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Board of Pharmacy Specialties Certification, required.
  • Additional Skills Required: Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
