*Cedar Sinai Park is looking for full-time and part-time Licensed Practical Nurses at our Skilled Nursing and Long Term Care community!*
*We are located at 6125 SW Boundary St. Portland, Oregon 97221, in the Raleigh Hills area of southwest Portland. We are on bus line 54 and 56 from Portland to Beaverton.*

*Summary*- Responsible for nursing care of assigned residents, under the direction of a Registered Nurse; to ensure the Cedar Sinai Park goal of providing care to our residents in an atmosphere of comfort, independence and dignity.
*Essential Functions*

· Supervises unit care of residents, assigns duties to Certified Nursing Assistants, evaluates completion of duties, and makes reassignments as necessary to ensure provision of service to the residents.
· Observes, documents and reports any significant changes in the resident's condition that warrants medical and/or nursing intervention.
· Assures the provision of nursing services and takes action to ensure fulfilling resident wishes and promoting the least restrictive alternative of care.
· Revises care plans daily as needed, by identifying problems, formulating measurable goals, and incorporating resident/family wishes and teaching; while respecting and promoting the resident to self-direct their own care.
· Administers medications to assigned residents as ordered by the physician, including intravenous services.
*Required Education, Experience, Licenses, and Certifications*
· Graduate of an accredited School of Practical Nursing required.
· Current, unencumbered license in the state of Oregon as a Licensed Practical Nurse required.
