POSITION OVERVIEW

 

Health Coaches are responsible for individual client’s care and are important contributors to Harbor’s team based care model. This Hybrid role will work directly with clients to understand what health means to them and assist clients in developing skills to navigate health challenges. Health Coaches work in lockstep with clinical providers to help clients develop, accelerate, and achieve health goals. With a focus on overall wellness, the Health Coaches main goal is to provide clients with the tools and knowledge to take control of their long term health.

POSITION DUTIES & RESPONSIBILITIES

 

Health Coaches are responsible for:

  • Conducting outreach to clients eligible for the program, to enroll them in services and provide guidance on how to reduce or eliminate high-risk behaviors
  • Maintaining a steady panel of clients, guiding them through protocols lasting four to twelve weeks
  • Adhering to a full-time work schedule and standard operating procedures to support success and connectivity between Harbor and its clients
  • Providing care over the phone, video visits, and in-person depending on client preferences while meeting program metrics
  • Developing customized programs to meet clients needs in diet, physical activity, sleep and stress
  • Assessing clients modifiable behaviors while providing, support, guidance and education on how to improve behaviors that put them "at risk" of chronic or disease conditions
  • Establishing goals with clients to improve health and quality of life, under provider’s guidance, informed by clinical data and evidence based practices
  • Receiving client questions, suggestions, and complaints with grace and empathy, while providing proactive solutions
  • Managing referrals, responding to patient cases, and documenting client interactions in Athena, Google Suite,TalkDesk and other relevant tools to support tracking program efficacy
  • Collaborating with the multi-disciplinary Health Coach Team to integrate best practices, assist with program needs assessments and implement pilots to enhance the client experience
  • Building trusting relationships with Harbor team members to assist with care coordination and ensure client’s needs are met
  • Being up to date on vaccinations
 

DESIRED PROFESSIONAL SKILLS & EXPERIENCE

  • Two years of relevant experience in a customer-facing coaching role
  • Strong interpersonal skills to effectively communicate with clients, clinicians, clinical support staff, product, and operations
  • Ability to work independently, leveraging organization and time management skills
  • Experience with medical terminology, medical office procedures, electronic medical records Additional skills & experiences preferred include:
  • 1 year of experience in a healthcare role (e.g., social worker, community health worker, case manager)
  • 1 year of experience in delivering behavior change interventions for specific lifestyle related issues
  • National Board for Health and Wellness Coaching certification
  • Bilingual, English and Spanish speaking
